Hospital Blood Bank Annual Report (HBBAR)

Regulation 12 of S.I No. 360 of 2005, European Communities (Quality and Safety of Human Blood and Blood Components) Regulations, requires that hospital blood banks submit an annual report to the Irish Medicines Board (IMB). Hospital blood banks are required to submit a declaration of compliance with the requirements of S.I. No. 360 of 2005 and provide details of the systems that are in place to ensure such compliance.

 

The Hospital Blood Bank Annual Report form has been developed by the team of Inspectors within the IMB that is responsible for Blood and Tissues.  The report forms are available on the IMB Website. Hospital Blood Banks are requested to return the completed forms to the IMB by 31st January of each year.
